Client Scenarios — Hospital-Based Physician Group Development/Management

INTENSIVIST CASE STUDY

Since launching our first Intensivist Management Program in 2002, Delphi now holds more than 4 contracts

Situation: Delphi was asked to work with the CEO of this particular hospital to offer consulting services for the implementation of an Intensivist Program.  For eight months, Delphi worked to determine the objectives of the program, build consensus with administration, provide a basic program description and construct protocols, prepare financial projections and establish physician candidate profiles and requirements.  During the consultation, the hospital decided to establish a long-term relationship with Delphi to design, recruit for, implement and manage the program.

Solution: Delphi recruited a Medical Director who was double boarded in Internal Medicine and Critical Care and had completed fellowships in Pulmonolgy and Cardiology.  Internal Medicine residents were used to help with first call coverage during nights and weekends.  Two full-time Intensivists with Delphi and two Pulmonologists on the medical staff share on-call responsibilities in backing up the Residents.  Our two Intensivists provide all weekday on-site coverage.

Outcome: Since implementing Delphi’s Intensivist Program this hospital has seen a significant positive financial impact due to cost savings, reduced Average Length of Stay in ICU and new incremental ICU admissions. Our client has 18 combined ICU and CCU beds and a 29-bed step-down unit overseen by our Intensivist Medical Director. Prior to the program the hospital Emergency Department, which averages 75,000 annual visits, was holding two to three patients at all times because no ICU beds were available. With Delphi’s Intensivist Program all admission delays have been completely eliminated.

ANESTHESIOLOGY CASE STUDY

Delphi launched our first Anesthesiology Program in 2004

Situation: Delphi assumed responsibility for the management and staffing of an eight full-time member Anesthesia Group.  The Anesthesiologists were providing servicess for the two facilities within our client's system. The goal was to restructure and strengthen Anesthesia coverage.

Solution: Delphi worked to secure exclusive arrangements with the existing eight physicians. We implemented a new compensation system with incentives, guaranteed the availability of coverage and facilitated some much needed changes in personalities within the group.

Outcome:  Both the hospital and physician group have seen many positive benefits since Delphi began managing the Anesthesiology Department. Scheduling of cases has been smoother since their is consistent availability of Anesthesia coverage.  In addition, having an exclusive group dedicated to the two facilities has resuled in a better working relationship with other members of the medical staff.
